Course Overview

A solidly implemented management system is the foundation for good business performance. The Risk Identification and Management module and the Continuous Improvement Loop are at the heart of the management system. Learning from failures plays an important part in that improvement loop. However, incident investigation to identify the relevant facts required for improvement is not easy. This workshop aims to explain relevant aspects of incident investigation and the required processes to get optimal information for learning.

Masterclass B: Certified Tripod Beta Practitioner Course

Course Overview

Incident analysis requires a basic knowledge of how the tools are applied as well as a special way of thinking. Moreover, it requires a basic understanding of human behaviour. The Tripod Beta tool aims to go deeper into the systems, beyond the immediate causes, deep into the reasons for the human behaviour and the real underlying causes that are at management system level. It has beendeveloped to create a “step-change” in learning from incidents. Tripod Beta is not limited to HSE incidents. It is focusing on “business process improvements.” This course contains all elements required to meet the Tripod Beta Practitioner accreditation criteria according to the Stichting Tripod Foundation. At the end of the course participants have the option to participate in the formal exam which is the first step of the formal accreditation process.

- Describe the context in which the Tripod Beta methodology fits

- Use Tripod Beta theory to construct accurate Tripod Beta diagrams.

For over a decade Bob has facilitated incident investigations using Tripod Beta Methodology. These included 28 fatalities, aviation incidents, armed robberies, hijacking, road tanker incidents and business process upsets. Bob has 25 years experience in oil and allied industries.
