Incident Investigation & Learning Masterclass

A solidly implemented management system is the foundation for good business perfor-mance. The Risk Identification and Management module and the Continuous Improvement Loop are at the heart of the management system. Learning from failures plays an important part in that improvement loop. However, incident investigation to identify the relevant facts required for improvement is not easy. When and how to start an investigation / Triggers / Terms of Reference / Positive and negative aspects of involving various parties in an investigation team? How to se-cure all relevant facts? How to discover the facts not disclosed by the incident report. This workshop aims to explain relevant aspects of incident investiga-tion and the required processes to get optimal information for learning.

    Tripod Beta Practitioner, Training Facilitator and by invitation of the Netherlands based Stichting Tripod Foundation an Appointed Tripod Beta Assessor.For over a decade Bob has facilitated incident investigations using Tripod Beta Methodology. These included 28 fatalities, aviation incidents, armed robberies, hijacking, road tanker incidents and business process upsets.Bob has 25 years experience in oil and allied industries. For 12 years he managed operational training in distribution activities for Shell Southern Africa.He holds a National Diploma  in  Safety  Management through  UNISA, with a  mechanical  engineering  background he has worked in  petrochemicals,  chemicals, road  tanker  operations,  and  construction. Since 1999  he  has  been  active as an independent consultant and has developed HSE Cases, investigated incidents and delivered operational and HSE learning events for contractors working for multinational oil companies. Commissioned by Shell International E&P, he incorporated the Hearts and Minds Programsinto  HSE  Workshops  for  Frontline  Supervisors  and  delivered  these  in  Europe, Africa, middle and far east.  Bob has preferred provider  status  with the  Energy Institute UK for his work with  Frontline  Supervisors  implementing  Hearts & Minds programs.
